ICD-10 in Switzerland
ICD-10 codes in Switzerland: what applies where
Whoever looks up an ICD-10 code usually has a second question too: do I have to at all, and in which form? The essentials in six tiles.

What ICD-10-GM is
ICD-10 is the WHO classification of diseases. The German Modification (GM) is the edition maintained in Germany and used in Switzerland. The Federal Statistical Office publishes the coding manual with the Swiss rules for it.

How a code is built
One letter for the chapter, two digits for the category, and after the dot up to two more for the specification. M54.5 is low back pain, E11.9 type 2 diabetes without complications, I10 essential hypertension, J45.0 predominantly allergic asthma.

Mandatory in hospitals for years
For inpatient stays, ICD-10-GM is binding. It is the basis of the SwissDRG case rates and of the federal medical statistics of hospitals.

In practices since TARDOC
TARDOC and outpatient flat rates have applied since 1 January 2026. Sessions with a trigger position are documented using ICD-10-GM. At the same provider, this also applies to other sessions for the same patient on the same day. Without such a session, the general rule provides for ICD-10-GM or the Tessin code.

Tessin code or ICD-10?
The Tessin code has a good two dozen coarse groups and sat on Swiss invoices for decades. ICD-10 is thousands of times finer and readable internationally. On a KVG invoice with a flat rate, only the first letter of the code and the chapter appear, not the full diagnosis.

And ICD-11?
The WHO's ICD-11 has been in force since 2022 but has not been introduced in Switzerland. The date is open and depends on the German-language edition. Until then ICD-10-GM applies, and this coder works with it.

Can I enter longer text or a whole document?
Yes, as pasted medical text up to 2'000 characters. A complete document text can be entered if it fits within that limit; select the relevant section of longer reports. The public tool does not provide file uploads. Do not enter personal identifiers and review the suggestions clinically.
Is the ICD-10 coder really free?
Yes. Unomed's free ICD-10-GM coder can be used at no cost, without licence fees and without an account. You enter a diagnosis and get the code.
Can I enter an ICD-10 code and look up what it means?
Yes. Enter the code, for example M54.5, and the coder shows its catalogue entry, in this case low back pain (“Kreuzschmerz” in the German-language catalogue), and the neighbouring codes. For a three-character category such as J45 you see the terminal codes below it. It works the other way round too: type the diagnosis in your own words and get the code.
How secure is my medical data?
For AI suggestions, your text is sent to a language model service. Please leave out any details that identify a person. For particularly sensitive content we recommend leaving out names and dates of birth and following the rules of your institution. For coding whole reports inside the practice workflow, Unomed Flows is the right tool, with data in Swiss data centres.
Can I rely on the suggestions?
The ICD-10 coder turns the entered text into coding suggestions. They have to be checked by the professional who takes them over. The tool replaces neither coding training nor the coding guidelines of your institution.
What is ICD-10-GM?
The WHO's International Classification of Diseases (ICD-10) in its German Modification (GM) is the catalogue used to code diagnoses in Switzerland: binding in hospitals for years, and in practices since TARDOC and the outpatient flat rates wherever a flat rate is billed. The codes serve not only billing but also the federal medical statistics, quality assurance and epidemiological analysis.
Which ICD-10 version is in use?
The tool produces suggestions using ICD-10-GM. The loaded annual version is currently not displayed. For use in Switzerland, the edition specified by the FSO for the billing year is authoritative. Check the code and the applicable coding guidelines before using it.
Do I have to register or install software?

Does the coder find codes even if I do not know the exact wording?
Yes, that is the point. You enter the diagnosis the way you would describe it, with abbreviations and practice terms, and the coder finds the matching entry in the catalogue. When several codes are possible, you see all of them with their labels and pick the most precise one.

Does my practice have to code in ICD-10 now?
Sessions with a trigger position require an ICD-10-GM diagnosis. At the same provider, this also applies to other sessions for the same patient on the same day. Otherwise, the general rule provides for ICD-10-GM or the Tessin code. KVG invoices using ICD-10 transmit the first letter and, for flat rates, also the capitulum. Flat rates under UVG, IVG and MVG require the terminal ICD-10 code; TARDOC also provides for the Tessin code with the MTK extension or a free-text diagnosis in those insurance areas. The relevant tariff agreements are authoritative.
